DVHHS CHIP Healthy Housing Survey
As part of the Des Moines Valley Health and Human Services (DVHHS) Community Health Improvement
Plan (CHIP), housing is identified in our top three priorities to work on. To keep the momentum going,
we are continuing to seek community input.
An assessment is currently underway, specifically focusing on Cottonwood and Jackson Counties. Help
shape a healthier future for our community. We are collaborating with local partners to identify and
address health inequities, focusing on housing, in Cottonwood and Jackson Counties.
Your anonymous feedback on this assessment will highlight community strengths and weaknesses,
guiding solutions to ensure everyone has the opportunity for better health. Information collected
